The weekly lesson plan is a grid: six subject rows across Monday to Friday, with the week-of date and a focus and reminders box at the top, and space underneath for materials and prep and for homework and assessments.
The grade book is one clean sheet for the whole class - class or period, subject and term at the top, then a row per student with columns for three assignments, a quiz, a test and an average.
The attendance and behavior page is a simple daily record: student by Monday through Friday with a notes column, plus a box for behavior and positive notes and one for students needing support - the two lists you actually reference at a parent meeting.
The last page is the follow-up. A parent and guardian contact log with date, student, the parent and the method you used, and the reason and outcome - the log that protects you when someone says nobody called. Plus this week's to-do, prep and copies to grab, reminders and important dates, and a notes and ideas box.
Five pages including the cover, 191 fill-in form fields, US Letter and A4 both included. Fill it on screen in any PDF reader, or print a fresh set each week.
The grade book and attendance sheets are generic and undated, so they work for any subject, any grade and any school year.
