The service agreement starts with what you will actually do - feeding, fresh water, walks and exercise, litter and waste cleanup, medication if noted, companionship - then the fee, the deposit that reserves the dates, and what a late cancellation costs. There are clauses for keys and access (key, code, lockbox or other, kept secure and returned or reset afterwards), for emergency veterinary care and reimbursement, and for liability, where the client confirms pets are healthy, vaccinated, and that behavior issues have been disclosed. Signature and date lines for both sides.
The pet and owner intake form is the one you have the client fill in before day one: owner name, phone, home address, an alternate emergency contact, then pet name, species and breed, age, weight, feeding amounts and times, medications and schedule, allergies and conditions, behavior notes, walk and exercise routine, commands and quirks, and the vet's name, phone and address.
The daily care and visit log is a row per visit - time in and out, fed and watered, walk and potty, meds, mood and notes. It doubles as the update you text the owner.
Four pages including the cover, 95 fill-in fields, US Letter and A4 both included. Editable, so you can put your own logo on it and use it as your business's paperwork.
