The essentials page is the one a doctor or an ER needs first: full name, date of birth, blood type, height and weight, primary doctor and phone, pharmacy and phone, insurance and policy number, emergency contact. Then three larger spaces - allergies and reactions, ongoing conditions and diagnoses, and surgeries and major history with dates.
The medication tracker is a row per item: medication or supplement, dose, when and how often, and the reason plus the prescriber. Underneath, a vaccination and immunization record.
The appointment log is a running history - date, doctor or clinic, reason and outcome, and the next step - with separate boxes for upcoming appointments and tests and for specialists and their contacts.
The symptom log is where patterns show up: date, symptom and severity, possible triggers, what helped. Facing it, a place to write questions for your doctor before the visit and notes or test results to remember after it.
Five pages including the cover, 104 fill-in form fields, US Letter and A4 both included. Fill it on screen in any PDF reader, or print it and put it in a binder.
This is a personal record-keeping tool, not medical advice. It organizes information you already have - it does not interpret anything, and it is not a substitute for your medical record. Because it holds your insurance number and your full history, keep the filled copy somewhere you would keep any other sensitive document.
